90 MINUTE EXTENDED EVALUATION
90 minute extended physical therapy evaluation is perfect for those with complex medical concerns such as hEDS, POTs, or chronic pain. This allows me to screen for these complaints or complete baseline testing to create a gameplan to get you on the journey toward your goals!
The first 10 miles are complimentary, any mileage over 10 miles from our location will be an additional $0.50 per mile to make this continued service feasible.

Our most popular package! Includes 60 minute initial evaluation and seven follow-up visits or 8 follow-up visits. It's our best deal like getting a visit free! All visits must be scheduled in same plan of care to optimize your wellness!

This option is the first time meeting, it serves as a way to create a plan and ask questions surrounding their complex medical journey through hEDS, POTs, Nutcracker syndrome, May Thurner syndrome, MALS, and Chronic Pain. This is a good option to prepare for a doctors visit and establish a plan.
30Â MINUTE VIRTUAL FOLLOW-UP
Can be chosen after initial call. This is your opportunity to tweak an already established plan, create a game plan for doctors visit coming up, ask questions to empower you to advocate for yourself.

MONTHLY SUBSCRIPTION
For a limited time, this includes a free 30 minute virtual call with your subscription! You have 24/7 access to send quick text message questions,  these will be answered during my next office hours which will be held Monday/ Wednesday/ Friday between 8-10am EST. I will try to answer questions more quickly, but as we will discuss, spoon management is very important!
All virtual calls are held on a a HIPPA compliant, two-way interactive video chat platform, but no guarantee can be made about internet connection or security of that platform.
16 VISITS
LSVT BIG is an intensive program where you are seen 4 times a week for 4 weeks. this includes a physical therapy evaluation where I will establish a Physical therapy diagnosis and we will discuss your goals for this time together as well as establish what tasks are most difficult for you. we will use the same core set exercises progressing through different tasks and settings in your home or the community to improve your confidence and indiependence!
